Return Authorization Requirement:

  • All items must have an approved Return Authorization before being returned.
  • Customers may return items within 3 days of purchase for a refund, excluding shipping costs, credit card processing charges and applicable restocking fees.

Restocking Fee:

  • 30% restocking fee will apply to all returned items without an approved Return Authorizations.

Credit Card Processing Fee:

  • 10% credit card processing fee applies to all returns to cover transaction costs.

No Show Fee:

We understand that sometimes unexpected events can arise, and we truly appreciate your understanding and respect for our time. However, when an appointment is missed without prior notice, it impacts not only our schedule but also other patients who are in need of timely care.

In order to maintain the quality of care and service we strive to provide, we have implemented the following no-show fees:

  • A $100 fee will be charged for missed appointments Monday through Friday.

  • A $200 fee will be charged for missed appointments on Saturdays.

These fees are nonrefundable and are in place to help us continue providing the best possible care for all of our patients. Our doctors make a dedicated effort to be on time for your appointment, and we ask that you kindly show the same respect by notifying us at least 24 hours in advance if you need to cancel or reschedule.
